Form is great…help from insurance companies gets worse every month. Never, ever been paid 1st time in 20 years. EVERY claim gets denied regardless, usually at least 3 times. On average, on phone for 8-40 hours to get paid. Company changes rule on some or many line items at random. It’s like a sport for them. Worst part of work…living nightmare. Would rather HAVE surgery.
